Anatomy of a Shoe: Every Part Named, and the Four That Actually Matter
The parts of a shoe explained by a foot and ankle surgeon: heel counter, last, stack, drop, toe spring, strobel — plus the four that decide how a shoe feels.

The short answer
A shoe is ten parts. Four of them — the heel counter, the midfoot, the toe box and the geometry underneath you — decide almost everything about how it feels. The rest is marketing surface: mesh, overlays, colourways, and the words on the box.
Knowing the names matters because the words a brand uses and the words a clinic uses are not the same words. “Arch support” is a marketing phrase. “Heel counter” is a part you can hold between two fingers and test in four seconds. This page is the vocabulary the rest of this site runs on.

The parts under your foot
Outsole
The rubber that touches the ground. It decides grip and how long the shoe lasts, and almost nothing else about comfort. Worn-through patches are the cheapest gait analysis there is: wear at the outer heel is normal, wear that has reached the foam underneath means the shoe is finished regardless of how it feels.
Midsole
The foam. This is the shoe. Everything people mean by cushioning, softness, bounce, firmness and “support” happens here. Two numbers describe it: stack height — how much material sits between your heel and the ground, and again under the forefoot — and drop, which is simply the heel stack minus the forefoot stack.
Drop is where brands and reality most often part company. Every shoe with a stated drop above zero that we have compared against independent lab measurements has measured a higher drop than the box states, and the reason is usually a thinner-than-stated forefoot rather than a taller heel. Lasting board, or strobel
The layer the upper is actually attached to, hidden under the insole. A board-lasted shoe has a stiff board there and feels planted and torsionally rigid. A slip-lasted or strobel shoe has the upper stitched to a flexible fabric sheet, and feels softer and more flexible through the middle. Almost no brand tells you which one you are buying, and it is the single biggest reason two shoes with identical specs feel completely different underfoot.
Insole, footbed, or top cover
The removable liner. In most shoes it contributes very little support and exists mainly to finish the interior — which is exactly why it comes out when a custom device goes in. Whether it lifts out at all, and how much material leaves with it, is the first thing we check in the Orthotic Volume Rating.
The parts around your foot
Upper
Everything above the sole. Modern uppers are mostly engineered mesh, which stretches in the directions the designer chose and not in the ones your foot may need. The upper holds your foot over the midsole; when it fails to, nothing the midsole does can rescue it.
Toe box
The front chamber. It has two dimensions that get confused constantly: width (how far across at the big toe and at the widest point) and depth (how much vertical room above the toes). A bunion needs width. A hammertoe or a thick nail needs depth. A shoe can be generous in one and mean in the other, and the box tells you neither. See wide toe box shoes for the measured numbers.
Vamp, throat and eyestay
The vamp is the panel over the top of your foot; the throat is the opening where it meets the lacing; the eyestay is the strip carrying the lace holes. Together they set instep volume — how much room there is over the arch of your foot. A shallow throat is why a shoe can be the right length and still feel like a clamp.
Tongue
The flap under the laces. Padding here is comfort, not support. A gusseted tongue (stitched to the sides) stops it sliding and is worth having; a thick unattached one is usually just bulk.
Collar
The padded rim around the ankle opening. It is the part people mistake for the heel counter. Squeeze it and you feel foam; the counter is underneath it, and it is not soft.
Heel counter
A stiff cup built into the back of the upper, wrapped around the rearfoot. It is the part that actually holds your heel in position, and it is the most important single component in the whole shoe for anyone with heel pain, flat feet or an orthotic. If it collapses under thumb pressure, the shoe is not doing the job whatever the marketing says.
Toe spring
The upward curve at the front of the sole. It lets the shoe roll forward without your toes having to bend as far, which is why a rockered shoe helps forefoot pain, arthritis of the big toe and a stiff first joint. Too much of it on a healthy foot can under-work the toe flexors.
The part you cannot see: the last
The last is the foot-shaped form the shoe is built around. It determines length, width, instep volume, heel-to-forefoot proportion and how much depth there is inside — that is to say, most of fit. It is never printed on the box and it changes between versions of the same shoe without the name changing.
This is the honest answer to “why does the new version of my shoe feel different?” Nothing you can see has changed. The form it was built on has.
The four that actually decide how a shoe feels
Everything above is vocabulary. These four are the test. Each takes about five seconds in a shop, and together they tell you more than any spec sheet.
- 1. Heel counter. Pinch the back of the shoe between thumb and forefinger, above the sole. It should resist. If the sides meet, there is nothing holding your heel.
- 2. Torsional rigidity. Hold the heel in one hand and the forefoot in the other and twist in opposite directions. A little give is fine. If it wrings out like a towel, the midfoot has no structure — which matters most for flat feet, posterior tibial tendon problems and anyone in a shoe all day.
- 3. Toe box width and depth. Take the insole out, stand on it, and look. If your foot is wider than the liner it sat on, the shoe is narrower than your foot. Then press down over the toes with the shoe on: you should feel space, not your nails.
- 4. The geometry underneath. Stack, drop and rocker. High stack with a rocker rolls you forward and unloads the forefoot; low stack and flat asks more of your own foot. Neither is better, but one of them is better for you. The drop chart and zero-drop shoes cover the trade-offs.
Notice that none of the four is cushioning. Softness is the thing shoppers test first and the thing that predicts comfort least.
Words brands use that do not mean what you think
- “Arch support.” Not a defined term and not a measurable part. Usually it means a shaped stock insole, which is the part you take out. What actually resists arch collapse is a stiff midfoot and a firm heel counter. See shoes for arch support.
- “Stability” and “motion control.” Category names, not specifications. A stability shoe from one brand can be softer and less torsionally rigid than a neutral shoe from another. Test the shoe in your hands, not the label.
- “Wide.” A width letter adds a small, brand-specific amount, and at least one major brand states that its narrow, medium and wide use the same footbed — only extra-wide widens the platform. The published figures are on the shoe width chart.
- “Orthotic friendly.” Means the insole is removable. It does not mean the shoe has the interior depth to hold a device once the insole is out, which is a different question — and the one the Orthotic Volume Rating exists to answer.
- “Zero drop.” A real, checkable specification, unlike the others here — but it describes geometry, not cushioning, and a zero-drop shoe can have a very high stack.
Frequently asked questions
What is the most important part of a shoe?
The heel counter, for most people with foot pain. It is the part that positions the rearfoot, and once the heel is not held the rest of the shoe cannot compensate. For forefoot problems the answer changes to the toe box and the rocker.
What is the difference between stack height and drop?
Stack height is how much shoe is under your foot, measured separately at the heel and at the forefoot. Drop is the difference between those two numbers. A shoe can be high-stack and low-drop at the same time; they are independent.
Is the insole the same as an orthotic?
No. The insole is the thin liner the shoe ships with, and in most shoes it is a finishing part rather than a supportive one. An orthotic is a device made to alter how your foot loads. The insole normally comes out so the orthotic can go in.
How do I know if a shoe is board-lasted or slip-lasted?
Lift the insole and look at what is underneath. A continuous fibreboard surface is board-lasted; visible stitching across a fabric sheet is strobel-lasted. It takes ten seconds and no brand will tell you.
Does a stiffer shoe mean a better shoe?
Stiffer in the right place. You want torsional rigidity through the midfoot and a firm heel counter, and you usually want flexibility at the ball of the foot unless a rocker or a carbon plate is doing that job deliberately. A shoe that is uniformly stiff is a cast; a shoe that is uniformly floppy is a sock.
Why does the new version of my favourite shoe feel different?
Almost always the last changed, or the midsole foam did. Neither is printed on the box, and the model name stays the same, so there is nothing visible to warn you. It is the single most common complaint we hear about footwear.
