The short answer
A supinating foot needs cushioning and shock absorption, not rigid arch support. This is the exact opposite of the advice given to flat feet, and it is where nearly every supination purchase goes wrong: people assume an insole means support, buy the firmest, tallest arch on the shelf, and make a rigid foot more rigid.
What you actually want is a device that fills the arch gap with something forgiving, cushions the heel and the outer forefoot, and lets the foot roll inward a little more than it naturally wants to.
Why a supinating foot hurts
Supination — underpronation — means the foot rolls outward and stays on its outer edge instead of rolling inward to absorb load. It usually goes with a high, rigid arch.
Pronation is how the foot absorbs shock: rolling in unlocks the joints and lets the arch flatten slightly, spreading the impact over time. A foot that does not do that is a stiff lever. The shock still exists — it just passes further up, and it concentrates on a narrow strip along the outside of the foot rather than being distributed.
That mechanism explains the pattern of problems that comes with it:
- Lateral ankle instability and repeated sprains — you are already balanced on the outer edge, so there is less margin before the ankle rolls.
- Peroneal tendon pain along the outside of the ankle, from tendons working constantly to stop that roll.
- Fifth metatarsal stress injuries — the outer forefoot takes load it was not designed to take alone.
- Heel pain and heel fat-pad soreness, from poor shock absorption at initial contact.
- Lateral knee and IT band irritation, as the load that the foot did not absorb travels up the chain.
The tell is the wear pattern. Stand your oldest pair of shoes on a table and look from behind: wear concentrated along the outer edge, from the heel through to the little-toe side, is the signature.
What to look for in the insole
1. Cushioning first
The priority is genuine shock absorption under the heel and the outer forefoot. Where a flat foot needs a semi-rigid shell that resists, a supinating foot wants a device that gives and returns. If your thumb cannot compress it at all, it is the wrong product for this foot.
2. Full-contact arch fill, not arch lift
A high arch leaves an air gap between the foot and the footbed, so all the load lands on the heel and the ball. The goal is to fill that gap so the middle of the foot carries some of the load — not to push the arch higher. The difference matters: a supportive shape that follows your arch contour and touches it gently is right; a firm shape that presses up into it is not.
This is also the strongest argument for a custom device in this foot type. A high arch is a specific shape, and off-the-shelf arch curves frequently miss it entirely — either sitting well below it, doing nothing, or catching it at a point.
3. Lateral posting, in some cases
A small wedge under the outer heel or forefoot encourages the foot to roll slightly inward and shares load off the outer edge. It is genuinely useful for recurrent lateral instability, and it is easy to overdo. This is the one feature worth having chosen for you rather than guessed.
4. A deep heel cup
Universal advice, and it matters here too. A high-arched foot often has a smaller, less forgiving heel fat pad, and a deep cup keeps what pad there is directly under the bone.
5. Volume
High-arched feet need vertical room — the instep is high, and adding a device can make a shoe feel tight across the top even when the length is right. Take the stock footbed out and check the fit over the instep, not just at the toes.
The shoe matters as much as the insole
Pair a supinating foot with a neutral, well-cushioned, reasonably flexible shoe. Stability and motion-control shoes are built to resist inward roll — which is the one thing this foot is already failing to do enough of. Putting a supinator in a stability shoe pushes them further onto the outer edge.
This is the same argument made from the shoe side in shoes for high arches and supination, and it is the reverse of the reasoning in overpronation.
The things that are not insoles
For lateral instability in particular, the insole is the smaller half of the answer.
- Balance and proprioception work. Single-leg stance, progressing to unstable surfaces, has a better evidence base for reducing recurrent ankle sprains than any insole.
- Peroneal strengthening — the muscles on the outside of the calf are what actively resist rolling out.
- Calf flexibility. A tight calf worsens almost every mechanical foot problem, this one included.
- Rotating two pairs of shoes, so the cushioning you are relying on has time to recover.
Who should skip these
- Anyone with no symptoms. A high arch that does not hurt needs nothing but a comfortable shoe.
- Anyone about to buy a firm, high-arch support device because it was marketed as premium. For this foot type that is the wrong tool, and it commonly produces new arch or outer-foot pain.
- Anyone with reduced sensation. A rigid high-arched foot with neuropathy has concentrated pressure points and cannot report them — that needs an accommodative device chosen by a clinician (shoes for neuropathy).
- Anyone with a high arch that is getting worse, or with weakness lifting the foot. A progressive high-arched deformity can have a neurological cause and deserves examination rather than an insole.
- Anyone with an acute ankle sprain. Treat the injury first; the insole is for afterwards.
