The fat pad under your heel bone is the only cushioning you were born with, and it is the one piece of shock absorption nobody can sell you. When it thins, the shoe has to do work the foot used to do by itself — and the specification that decides whether a shoe can do that work, the hardness of the foam, is not printed on a single box in the shop. So this page does the only honest thing available: it puts the laboratory-measured heel stack height and the laboratory-measured foam hardness of eight shoes side by side, and shows that you cannot read one from the other.

Is it the fat pad, or is it the fascia?
This matters more than the shoe does, because the two conditions want opposite things from a sole. Plantar fasciopathy wants the fascia unloaded — an arch that is supported, a heel that is raised, sometimes a rocker. Heel fat pad syndrome wants the impact absorbed and the pad held in place, and an aggressive arch support can make it worse by tipping load back onto a heel that cannot take it.
A 2022 scoping review pooled what evidence exists on telling them apart. These are the pain behaviours that shifted the odds, and they are worth reading as a checklist rather than a diagnosis:

The other difference is where it hurts. Fat pad pain sits in the middle of the heel, under the weight-bearing part of the bone, and it is worse barefoot on a hard floor than in a shoe. Fascial pain sits further forward and toward the inside edge, where the fascia attaches. What the shoe actually has to do
- Soft foam directly under the heel. The pad has lost some of its ability to absorb and spread the impact of heel strike. Something has to. Foam hardness — not stack height — is the property that does it.
- A heel counter that actually holds. A healthy fat pad is a sealed set of fibrous chambers that stops the fat squirting sideways under load. A degraded one splays. A firm, well-fitted heel counter is the closest a shoe gets to replacing that containment, which is why a collapsed or floppy back on an old shoe makes this condition worse.
- Enough depth to take a heel cup if you need one. A viscoelastic heel cup is the standard first suggestion for this diagnosis. It only works if the shoe has the volume for it, which means a removable insole, not a glued-in one.
- No aggressive arch prop. A tall medial support tilts load laterally and posteriorly. For a fascia that can be the point; for a thinned heel pad it usually is not.
Three of those four are properties nobody publishes. Brands publish stack height sometimes, drop usually, and foam hardness never — and heel-counter stiffness only appears at all because an independent laboratory grades it. That is the gap this page exists to close, and it is why the numbers below come from a lab rather than from a marketing page.
A taller heel is not a softer one

Eight shoes is not a study. It is, however, enough to kill a shortcut. Across these eight the correlation between heel stack height and midsole softness is r = −0.06, which is no relationship at all. The tallest heel here — the HOKA Clifton 10 at 44.4 mm measured — sits in soft foam. The shortest — the Brooks Addiction Walker 2 at 35.7 mm — has the firmest foam of the group. And the shoe with a mid-height heel, the Glycerin 23, has the softest foam on file. Buying “the tallest one” is buying the wrong number.

RunRepeat lab Softer is lower on the Asker C scale. “Not measured” means the laboratory did not grade that shoe’s heel counter, not that the counter is poor. We have not verified any of these numbers ourselves.

Brooks Glycerin 23 Spec analysis
On the two properties this diagnosis cares about, the Glycerin 23 is the best-documented shoe on the site. Its midsole measured 29.2 on the Asker C scale against a laboratory average of 35.6 — genuinely soft, not soft-for-marketing — and the same laboratory graded its heel counter 4 out of 5. Soft underneath, firm around the outside, which is the combination a thinned pad wants and the combination almost nothing else on this list has in both halves. The insole is removable at 4.4 mm, so a heel cup displaces very little.
Best suited to: mid-heel pain that is worse after standing, in someone who wants the softest documented foam and may add a heel cup.

Brooks Adrenaline GTS 25 Spec analysis
The interesting thing about the 25 is what changed. Its predecessor measured 44.1 Asker C — firm; the 25 measured 34.5 — soft, on the same durometer, with the same 4-of-5 heel counter. For a runner who wanted a firm guided platform that is a downgrade, and our GTS 24 analysis says so. For a thinned heel pad that also pronates, it is the shoe that got better: guidance from the GuideRails rather than from a hard midsole, and a 4.9 mm removable insole that leaves room for a cup.
Best suited to: heel-pad pain in a foot that also rolls in, where a firm stability shoe has been tried and hurt.

HOKA Clifton 10 Spec analysis
The Clifton is the one shoe here that gives you height and softness: 44.4 mm of measured heel stack at 34.2 Asker C, and at 275 g measured it is light for the amount of foam involved, which matters if you are on your feet all day. The honest caveat is the other half of the brief. Nobody has graded its heel counter, and a soft midsole on a 44 mm heel is a lot of shoe between you and the ground — if you already feel unsteady, that is a real trade, not a theoretical one. Note also that HOKA publishes an 8 mm drop and the laboratory measured 12.4 mm.
Best suited to: long days on hard floors where impact is the problem and stability is not.

The shoe you were probably about to buy
The HOKA Bondi 9 is the shoe most often recommended for heel pain, and on the number that matters here it is not what people think it is. Its midsole measured 43.0 Asker C — firmer than the Clifton 10, firmer than the Glycerin 23 by nearly fourteen points, and firmer than the Bondi 8 it replaced, which read 41.5. The Bondi did not get plusher when it got taller. It got taller and slightly firmer.
That does not make it a bad shoe, and for someone who wants maximum material between foot and floor it may still be the right one. It does mean that “get a Bondi” is advice based on how the shoe looks rather than on how the foam behaves, and if a Bondi has already failed to help your heel, the reason may be that you needed softer rather than taller.
The one we would not choose for this diagnosis
The Brooks Addiction Walker 2 is an excellent motion-control walking shoe — a 5-of-5 torsional rigidity, a leather upper, four widths, and a genuine place in our overpronation and Charcot foot guides. It is also the firmest foam on this site at 48.7 Asker C, on the shortest measured heel stack of the eight at 35.7 mm, with a heel counter graded 3 of 5. Firm, low and containing-but-not-the-firmest is close to the opposite of the brief here. Right shoe, wrong diagnosis.
What the research actually supports — and where it stops
Everything above this heading is clinical reasoning about published measurements. This is the part with evidence behind it — and the part that is still judgement.
There is exactly one synthesis of the evidence on this condition. Chang, Rasmussen, Jensen, Sørensen and Rathleff searched eight databases plus two trial registries and published a scoping review of heel fat pad syndrome in the Journal of Foot and Ankle Research in 2022. They screened 1,668 records, read 95 in full, and found seven original studies in the entire literature.
- This may be the second most common cause of pain under the heel. Among patients presenting with plantar heel pain, 15% had heel fat pad syndrome against 53% with plantar fasciopathy; in community-dwelling older adults the prevalence was 4.2%.
- The pain pattern does distinguish it. Worsening pain on prolonged standing, pain at night and bilateral pain each raised the odds of fat pad syndrome by roughly twenty-fold; morning first-step pain and tenderness on the medial calcaneal tuberosity all but ruled it out.
- Ultrasound helps, but no single number diagnoses anyone. Unloaded pad thickness averaged 7.23 mm in cases against 10.36 mm in controls, and a threshold of 9 mm or less gave 85.5% sensitivity and 82.2% specificity — against a healthy range that runs from 12 to 29 mm.
- The treatment everyone recommends has never been trialled. In the authors’ words, randomised controlled trials of viscoelastic heel cups or arch taping for this condition do not exist. The two interventional studies they found were a single case report and one uncontrolled study.
The authors are direct about the limits, and we will not soften them. “Only 7 studies were included in the final data, which limits our ability to categorically synthesize evidence,” they write, and “subpar scientific rigor in some of the included studies also dampens our confidence in part of the summarized evidence.” They also excluded studies of the heel pad in healthy people and in people with plantar fasciopathy, so the review describes the condition rather than the tissue.
What that means for this page: the diagnostic checklist above has real evidence behind it, and everything about which shoe does not. No trial has compared one shoe against another for a thinned heel pad, and nobody has tested whether a softer midsole helps it. The shoe reasoning here is mechanism, not proof — the same honest position we take on the fasciitis guide. What the measurements can do is make sure that when you choose on mechanism, you are at least choosing on the right number.
Questions people ask
Does the heel fat pad grow back?
Not meaningfully. The pad is a structured honeycomb of fat held in fibrous chambers, and once those walls break down the tissue does not reorganise itself. That is why management is about offloading and substitution rather than repair, and why the shoe matters more here than it does for conditions that heal.
Are heel cups worth buying?
They are the standard suggestion and they are cheap, so trying one is reasonable. Be clear about what you are buying, though: as of the 2022 review there is no randomised trial showing they work for this condition. A cup does two things mechanically — adds material and walls the pad in — and the walling is probably the more important half. It needs a shoe with a removable insole to fit without cramping the foot.
Should I stop going barefoot at home?
For most people with this diagnosis, yes — barefoot on tile or hardwood is the single hardest thing you do all day, and unlike a walk you cannot shorten it. A cushioned house shoe is one of the few changes that costs nothing and applies for hours. Our slipper guide covers what to look for indoors.
Is a zero-drop or minimalist shoe a bad idea?
For a thinned heel pad, generally yes, and for a simple reason: a minimal shoe deliberately reduces the material between the heel and the ground, which is the material you are short of. Why do two shoes with the same stack height feel so different?
Because stack height describes how much foam there is, not how the foam behaves. The Ghost 17 and the Adrenaline GTS 25 measured 36.2 mm and 36.1 mm at the heel — a tenth of a millimetre apart — and 36.2 against 34.5 Asker C, which is a noticeable difference underfoot. Only one of those two numbers is ever printed, and it is the less useful one.
Can a shoe fix this on its own?
No. Footwear changes the load; it does not restore tissue. If heel pain has lasted more than six weeks, is waking you at night, or follows a fall or a steroid injection into the heel, that is a reason to be examined rather than to buy another pair of shoes. Ultrasound can measure the pad in a few minutes.
